This support is required in order for individuals to lead more fulfilled lives within their own designated living quarters. If you do not want to live in a facility that provides residential care but you would have trouble managing day-to-day responsibilities, this service is the ideal solution for you.

If you decide to sign into our supported living services, you will be given your own living space, each of which will have its own lease agreement. You will be given the option to live by yourself or to share your living space with other people. Either way, the decision is entirely up to you.

Houses, apartments, and fully individualised living arrangements are among the many kinds of accommodation that we have available for rent. You and/or the people you care about can master the skills necessary for daily life with the help of assistive technology services that are available in certain of our homes.

Autism

To better support people with autism and assist them in the development of functional abilities and skills that will enhance the quality of their lives, Total Healthcare has implemented a customised strategy that involves people with autism, as well as their families and other professionals in the healthcare industry.

This makes it possible to make a personalised treatment plan, which can then be reviewed regularly to keep up with a person’s growth and change.These plans include how each individual should be supported, their preferences, how to make their environment suitable for them, and how to make sure that all of their requirements, whether physiological, physical, psychological, or emotional, are met.

We take into account the recommendations made by psychologists as well as those made by occupational therapists, speech-language pathologists, and speech-language pathologists. We foster self-esteem and independence.

Mental Health Needs

Our support can range from a few hours a week to full-time or considerable support over a relatively short period of time, depending on the requirements. This is done with the intention of supporting the accomplishment of a certain objective. Physical Disability

We help people with physical limitations of all kinds, whether temporary or permanent, inherited or acquired. We have also worked with people with cerebral palsy, spinal cord injuries, amputation, multiple sclerosis, spina bifida, musculoskeletal injuries, and muscular dystrophy.

Many persons with physical impairments also have other health concerns such as learning difficulties, brain traumas, mental health care needs, and other medical needs. Our professional employees have been trained and have assisted clients with a wide range of complex requirements.

We provide services ranging from a few hours a week to 24 hours a day, seven days a week. Our programmes are always individualised for each individual and those who are important to them, ensuring that we understand the problems they’ve previously conquered as well as their present needs and aspirations and long-term ambitions.

Our Support Plans:

We ensure that everyone receives a personalised care and support plan over which they have control and with which they may engage with our staff and their family. Our support programmes are based on the Department of Health-approved Outcomes Star Model. The Outcomes Star Model is concerned with the following aspects:

  • Managing one’s mental health
  • Enhancing living abilities
  • Creating and sustaining social ties
  • Developing self-reliance

The following will be included in the support plan:

  • Tenancy and financial management.
  • Spending time with family and friends.
  • Assistance with appointment attendance
  • Hobbies or activities that you find enjoyable.
  • College enrollment, employment placements, and training opportunities
  • Advice and representation
  • Domestic and living skills are being developed, such as cooking and cleaning.
